How do I file a claim?

You must be the account owner or account administrator to file a claim. You will need your Verizon account PIN. If your device: Lost, stolen or damaged: Note:

You must pay Asurion the applicable deductible at the time of reporting the claim. File a claim on Asurion's website Call Asurion at (888) 881-2622 File a claim through the My Verizon app: Tap the menu icon in the upper left corner. Tap Devices.

Scroll to the device that was lost, stolen or damaged and tap Manage Device.

Tap Lost, stolen or damaged device? Start a claim. Enter the requested information and follow the on-screen instructions to complete the filing of your claim. Has a fault within or after the warranty period: Check My Verizon to see if you are eligible for repair or call Verizon. Florida customers: please file a claim on Asurion's website or call (888) 881-2622 if your device stops working due to a mechanical or electrical failure after the manufacturer's warranty expires.
