The film is based on the one-acters 27 Wagons Full of Cotton (1946) and The Unsatisfied Supper. (1948) by Tennessee Williams and is about a nineteen-year-old girl who behaves like a baby. The term 'babydoll' for a short, thin and transparent nightgown was introduced and popularized through this film. Actress Carroll Baker, who plays Baby Doll, wears such a nightgown in the film.

Although the film was approved by the Production Code Administration (PCA), (Hollywood's self-censorship for sex and drugs in films), the film was considered controversial and almost banished from cinemas. 77% of cinemas refrained from screening and the film brought only 600,000 dollars on. According to Kazan, the film didn't make a profit. There was appreciation in the form of a Golden Globe for Elia Kazan as director and a BAFTA for Eli Wallach as the most promising newcomer. There were also four Oscar nominations, which, however, were not redeemed.
