Zwangerschapsdiabetes - Zo hou je het onder controle


Waarschijnlijk ben je op deze pagina belandt omdat je net te horen hebt gekregen dat je zwangerschapsdiabetes hebt. Vaak is dat even flink schrikken, ik weet het want ik had het zelf ook nooit verwacht. Toch werd er bij mij na 26 weken zwangerschapssuiker vastgesteld tijdens een GTT (glucose tolerantie test). En dan begint het hele circus.

Afspraken met de gynaecoloog, internist, diabetesverpleegkundige, diëtist, extra echo’s, er komt ontzettend veel op je af. En als je dan zoals ik ook nog eens bang bent voor alles wat met spuiten en prikken te maken heeft dan ben je voor je gevoel in een nare droom terecht gekomen waar je maar niet uit wakker kunt worden. Zeker als de mogelijke spuiten met insuline in bijna alle gesprekken genoemd worden.

Maar het hoeft niet zo te lopen! Er is veel dat je zelf kunt doen om zwangerschapssuiker goed onder controle te houden, zodat insuline spuiten niet nodig is. Ik heb gedurende mijn zwangerschap veel tips en adviezen gekregen van mijn verloskundige, gynaecoloog, internist, diëtiste en diabetesverpleegkundige en hiermee heb ik de insuline spuiten buiten de deur kunnen houden. Nu wil ik deze tips en ervaringen graag met jullie delen zodat jullie hopelijk ook je zwangerschapssuiker goed onder controle kunnen houden. 

Let wel: Ik heb deze tips van medisch specialisten gekregen, maar ik ben zelf geen medisch specialist! Ik adviseer je dan ook voordat je deze tips opvolgt navraag te doen bij je behandeld arts(en). Neem bij vragen, twijfel of klachten contact op met je diëtiste en/of diabetesverpleegkundige.

Tip 1: Verspreid je voeding goed over de dag

Ik hanteerde 6 eetmomenten op een dag om m’n voeding goed te verspreiden. Dat zag er als volgt uit:

  • Ontbijt: 2 volkoren boterhammen met hartig beleg, koffie zonder suiker
  • Tussendoor: 1 zure appel of mandarijntje
  • Lunch: 2 volkoren boterhammen met hartig beleg, beker magere melk
  • Tussendoor: 2 volkoren crackers met hartig beleg, kopje bouillon of thee zonder suiker
  • Avondeten: 1 normaal bord avondeten, geen tweede keer opscheppen wat ik eerst wel vaak deed
  • Tussendoor: toetje in de vorm van een schaaltje magere yoghurt of kwark met fruit (en eventueel wat zoetstof)

Voor jou werken misschien andere voedingsmiddelen, maar zorg in ieder geval dat je regelmatig eet. Als er minder tijd tussen je eetmomenten zit door middel van tussendoortjes dan blijft je bloedsuikerspiegel stabieler.

    Tip 2: Eet alleen volkoren producten

    Dus alleen volkoren brood, crackers, beschuit en pasta. In de niet-volkoren varianten zit namelijk veel meer suiker en dus stijgt hierdoor je bloedsuiker meer. Van de volkoren varianten kun je meer eten en hou je langer een verzadigd gevoel en je bloedsuiker blijft een stuk lager. Eet in plaats van gewone rijst ook zilvervliesrijst, dit geeft ook langer een verzadigd gevoel.

    Tip 3: Vermijd snelle suikers


    Ongetwijfeld heeft je diëtiste je dit ook al verteld, maar ik vermeld het toch nog maar een keer: Geen snelle suikers! Hoera! Nee...? Geen hoera? Oké... het is ook niet leuk, maar wel noodzakelijk... én je kan het!

    Vooral alles wat je drinkt met veel suiker wordt razendsnel door je lichaam opgenomen en verhoogt je bloedsuikerwaarde enorm. Zonde dus! Neem van alle frisdranken de light variant, behalve als je Rivella drinkt want daar zit echt niets in en vond ik toevallig ook nog eens verslavend lekker. Drink geen vruchtensappen of limonades en neem geen suiker in je koffie of thee. Doe in plaats daarvan een zoetje in je koffie of thee en gebruik Slimpie, dat is limonade siroop zonder suiker.

    Vermijd verder uiteraard zoveel mogelijk dingen als koek, snoep, taart, drop (ook voor je bloeddruk!) en zoet broodbeleg. Let op: pindakaas is geen zoet broodbeleg en kun je prima eten. Niet leuk voor de zoetekauwen onder ons, maar goed nieuws voor de (pinda)kaaskoppen.

    Tip 4: Eerst eiwitten dan koolhydraten

    Van mijn verloskundige kreeg ik de tip om tijdens een maaltijd eerst wat eiwitten te eten en dan pas de koolhydraten. Dit zorgt ervoor dat de bloedsuikerwaarde minder stijgt. Dus neem bij het avondeten eerst een paar happen van je vlees en daarna pas van je aardappelen, pasta of rijst. En bij de lunch kun je bijvoorbeeld eerst je glas melk drinken en dan pas je boterhammen eten.

    Tip 5: Eet niet te laat op de avond

    Eet je laatste tussendoortje (in mijn geval was dat mijn toetje) ongeveer 11 à 12 uur voordat je je nuchtere waarde moet meten in de ochtend. In mijn geval mat ik m’n nuchtere waarde meestal rond 9 uur ’s ochtends en dus at ik m’n toetje zo tussen 9 en 10 uur ’s avonds. Zo heb je toch nog wat te eten na je avondeten, maar zit er genoeg tijd tussen je laatste eten en je eerste meting van de volgende dag.

    Tip 6: Zorg dat je laatste tussendoortje van de dag iets zuurs bevat

    Als je last hebt van een hoge nuchtere waarde in de ochtend terwijl de rest van de dag je bloedsuikerwaardes normaal zijn dan kan het zijn dat je last hebt van het ‘Dawn Syndrome’ of ‘Dageraad Syndroom’. Heel frustrerend want je eet ’s nachts niets en dus lijkt daar ook niet zoveel aan te doen. Je lever maakt ’s nachts suiker aan om ervoor te zorgen dat je bloedsuiker gedurende de nacht niet te ver daalt. Maar als je lever hier iets te enthousiast in is dan kom je dus ’s ochtends te hoog uit.

    Wat kan helpen is iets zuurs te eten voor het slapen gaan. Het zuur zorgt ervoor dat je insulinereceptoren beter werken en dat er dus minder insuline nodig is om je bloedsuiker binnen de norm te houden. Denk bij iets zuurs bijvoorbeeld aan een schaaltje yoghurt, een zure appel of een paar augurken. Niet allemaal tegelijk natuurlijk, maar kies er één en probeer dat een aantal dagen om te kijken of het effect heeft. Bij mij werkte zowel de yoghurt als de augurken (of een zure haring!) erg goed.

    Tip 7: Ga bewegen!

    Jawel mensen, ik heb het gezegd. De vrouw die alleen maar zittende hobby's heeft en die een broertje dood heeft aan de sportschool zegt: ga bewegen! En dan bedoel ik niet dat je hoeft te gaan rammen op allerlei apparaten in de sportschool, maar ga wel een stukje wandelen, fietsen, zwemmen, de hond (van de buren) uitlaten, lopend naar de supermarkt, verzin het maar.

    Mocht je naarmate de weken verstrijken toch merken dat je bloedsuikerwaardes wat op beginnen te lopen, dan kun je altijd nog terugvallen op brood en crackers met minder koolhydraten.

    Heb je al een kindje zoals ik, ga er dan lekker op uit naar de speeltuin, de kinderboerderij of de eendjes voeren. Je kind heeft lol en zo wordt het bewegen voor jou ook een stuk leuker. En is het een keer snertweer? Dan zette ik (niet lachen!) Nederland in Beweging op en ging ik midden in de huiskamer met m’n zoontje van twee staan gymmen met Olga en Duco. Geen gezicht ongetwijfeld... en nee, er bestaan geen filmpjes van (hoop ik). 

    Tip 8: Was goed je handen

    Prik je bloed en schrik je je een hoedje omdat je waarde ineens veel te hoog is? Dan heb je misschien, zoals als ik, net een fruithap voor je oudste klaar staan maken en niet (lang of goed genoeg) je handen gewassen voor je meting. 

    Het is me een paar keer gebeurd dat ik de suikerwaarde van een aardbei of een banaan aan het meten was en geloof me... die is te hoog! Na nog een keer uitgebreid handen wassen viel de meting beduidend lager uit. Eigenlijk is de tip dus, eerst meten en dan fruithap klaarmaken. 

    Tip 9: Meet je nuchtere waarde direct na het opstaan

    Wacht niet te lang na het opstaan met het meten van je nuchtere waarde ’s ochtends. Het lichaam maakt bij het ontwaken namelijk het hormoon Cortisol aan. Dit zorgt ervoor dat je wakker wordt en wekt het hongergevoel op. Cortisol wordt ook wel ‘het stresshormoon’ genoemd omdat je lichaam dit hormoon ook bij stress produceert.

    Vroeger, heel heel ver vroeger, was dit hormoon ’s ochtends nodig omdat het vergaren van een ontbijt betekende dag je het gevaar op moest zoeken. Tegenwoordig betekent het vergaren van een ontbijt niet veel meer dan de koelkast opentrekken en wordt de Cortisol dus nergens meer voor gebruikt. En wat doet het lichaam met Cortisol dat niet gebruikt wordt? Dat zet het om in glucose... handig. Dus als je langer wacht na het opstaan met meten, dan ben je misschien nog wel nuchter maar je lichaam is bezig om van de Cortisol suiker te maken en dus kan je meting daardoor hoger uitvallen. Het beste is meteen na het opstaan even te meten en pas daarna te gaan douchen, aankleden en al het andere.

    Tip 10: Vermijd stress

    Dit is echt een lastige, ik weet het. Vooral als de insuline spuit voor je gevoel als het zwaard van Damocles boven je hoofd hangt. Maar weet dat je er zelf nog best wat aan kunt doen om dat zo lang mogelijk uit te stellen en in veel gevallen zelfs helemaal te vermijden. Mijn internist had niet gedacht dat ik met mijn gewicht en GTT uitslag zonder insuline af zou kunnen, maar het is toch mooi gelukt! 

    Zorg dus naast voldoende beweging ook voor voldoende ontspanning. Ga leuke dingen doen, luister muziek, kijk een film, doe wat jou ontspant. Zo hou je de aanmaak van het stresshormoon Cortisol laag en blijven je bloedsuikerwaardes dus ook lager.

    Ook als er bij de extra echo’s die je krijgt gezegd wordt dat je kind voorloopt op groei, niet stressen. Neem het ter kennisgeving aan, ga door met je dieet en maak je er niet teveel zorgen over. Echo’s zijn geen exacte metingen, zeker naarmate je zwangerschap vordert. Een kind dat een beetje schuin ligt geeft al een verschil van enkele millimeters meer op een echo en dan worden ze volgens de grafiek al snel te groot.

    Mijn jongste zoon zou zo’n 4500 gram gaan wegen volgens de echo’s, en z’n buikomvang liep tot wel 6 weken voor in groei. En wat denk je? Hij woog bij de geboorte een keurige 3665 gram en had in de verste verte geen dikke buik. De verloskundige riep verbaasd toen hij tevoorschijn kwam ‘Oh, je bent helemaal niet groot!’. Tijdens mijn eerste zwangerschap (zonder zwangerschapssuiker) dachten ze dat mijn zoon maar liefst 5000 gram (!) zou gaan wegen en ook hij woog een nette 3860 gram en was lang maar helemaal niet dik. Kortom stressen over de gewichtsbepaling bij de echo’s heeft geen enkele zin, het is toch echt een beetje koffiedik kijken, zeker in de laatste weken.

    Tip 11: In geval van nood, koolhydraatarm brood

    Dat rijmt niet alleen, het is ook heel handig. Mocht je naarmate de weken verstrijken toch merken dat je bloedsuikerwaardes wat op beginnen te lopen (omdat het zwangerschapshormoon toeneemt) dan kun je altijd nog terugvallen op brood en crackers met minder koolhydraten. Ik zou hier zeker niet meteen mee beginnen, eerst lekker gewoon volkoren brood eten, maar het is goed en geruststellend om achter de hand te hebben voor als het nodig is.

    Er bestaat bijvoorbeeld volkoren brood en crackers van Atkins, met 65% minder koolhydraten. Ook kun je van dat merk volkoren koolhydraatarme pasta krijgen. Het is niet goedkoop, dus nog een reden om er niet mee te beginnen als het niet nodig is, maar weet dat je het achter de hand hebt.

    Met deze tips heb ik mijn zwangerschapssuiker prima onder controle weten te houden en heb ik geen insuline hoeven te gebruiken. Mijn zoon had een keurig gemiddeld geboortegewicht en is kerngezond. Ik hoop dat de tips jou ook zullen helpen en mocht je nog vragen hebben stel ze dan gerust hieronder. Oh ja, en mijn laatste tip: vergeet niet ook nog te genieten van je zwangerschap, voor je het weet is het voorbij.

This time the people from the street did not take it anymore, Jan with the cap no longer believed in the promises of yet another reform of the financial sector, no longer trusted in the laws imposed by the institutional powers on the banks. The taxes that were raised for the umpteenth time remained stuck in the throat of the citizen. They swallowed for the umpteenth time, but now they spit out government reforms. Too much is too much! The poor became poorer and the rich had a monopoly position in both the Eurozone and the rest of the world. First there were protests on a small scale. However, they grew as an enormous momentum in just a few weeks into a cascade of deadly waterfalls clapping down in the form of uncontrolled destruction and inhumane violence. Worldwide, the stock markets crashed and banks were bombed by people who wanted to secure their hard-earned savings in vain. In some countries, bloody unrest broke out, sometimes growing in certain regions into outright civil war. As a result of this unrest and insurrection, some of the population were thinned out in the first phase. Violent looting by both adults and young people from all classes of the population were daily costs. What one did not have, they stole. What one wanted, one took. Sometimes with a hard hand! Rape, public executions without trial were numerous and many times for banal reasons. Both on Heads of Government who had committed crimes against humanity and on as many innocent victims who happened to be in the wrong place at the wrong time. Lawlessness was in power and chaos preached his creed. Governments were knocked down. In some countries, part of the army joined the population. There was fighting with conventional weapons but here and there in a growing madness a button was pushed several times and cities disappeared from the face of the earth as if they had never existed. Tsunamis flooded the low-lying countries. Those who lived at sea level or lower were birds for the cat. When the violence of the water departed, the mainland was a large graveyard. In addition to this reaction of nature to the impact of the nuclear bombs, there was also the fallout that caused a decimation of the population. The problem of overpopulation in China was solved in a few months. The fatal result of the bombing caused a mass of victims. The fallout took care of the rest of the dead. For them it was a slow and painful death. A nuclear winter emerged in much of the world! Extensive zones were quarantined and survivors were left to their fate. In the end, a large part died from the effects of radiation or simply from deprivation. It was hell on earth, the devil was in power and his kingdom grew visibly. But just as many times during the evolution of man there were some people with some common sense, also individuals who were partly immune to radiation because of experimentation with special drugs and injections or simply lucky. People who joined in small groups in the aftermath of the devastation, formed new communities and grew into new nations. This happened both in the West and in the East. There was a period called the 'Second Cold War', a period of distrust and misunderstanding between the two zones and, as usual, the blame for the devastation was invariably placed on the other party. The map of the world was re-colored. As a result of the nuclear attacks, there were earthquakes, eruptions of new and old volcanoes, landslides and new islands were born. Where the East China Sea as part of the Pacific was used to be a natural border between China and Japan, these nations were now connected. The tectonic plates had pushed the seabed between these two nations upwards and now the two states were connected by land. China was so depleted in terms of its population as a result of nuclear explosions that, in addition to the people who had emigrated to Korea or Japan, there was not much left of the world power they used to be. The 21st century local disputes around the oil wells in this area were now one of their least problems. Whereas in the 15th century North and South America had been named the New World because of the discovery by European explorers of these new quotas, this was the exact opposite in 2112. After the Great War as the conflict had been called, the people of the New World in the East licked when the Old World in the West. It became a time when there was an atmosphere of irreparable distrust. There were smaller conflicts and mutual reproaches. One party blamed the other party in each confrontation anyway. The Old World with America as a precursor and Japan and China as the main representatives of the New World in the East realized after a few decades that it could not go on like this. It was decided that the quarantine zones that are Deeplands , because of the thousands of craters in the area between the two worlds, an obvious natural boundary between the two superpowers would become. That border ultimately created a fragile and armed peace. So it went for decades. Generation after generation, the lessons of the war years and their consequences were made up as a compulsory subject in education. Both worlds were rebuilt step by step. New governments came to power. New and better laws were voted and gradually rapprochement between the two worlds. First by some people, ordinary people who came into contact with each other via the restored Internet, despite the ban on government and the controls on various means of communication. Seeds were planted in fertile soil and they grew, albeit slowly, but they became mature in their quirky way that is peculiar to life,. Brittle as they were in the beginning, it was again a probe of borders, a rediscover and appreciate new cultures and thought. There was hope again and hope does live. Tokyo, the eastern city got a big sister at its borders. They chose the very appropriate name for the new city and that was' Sanctuary '. This is contrary to the past few years, which were marked by destruction and a constant sense of insecurity, an ever-present awareness of threat among the population. ' Sanctuary 'became the new capital and had to turn this supermodern capital of the New World into a place where people felt especially safe after all those years of war violence. As the meaning of the word Sanctuary indicated, the name of the city stood for “refuge” and “security”. It possessed in all its facets the latest technology and applications in the fields of science, architecture, ICT, robotics with its specific applications in home automation and many other technological advances. Hundreds of architects had spent years looking at this project, and after a period of destruction, this new building, this modern creation, was a crown on the head of the power that drove Homo Sapiens Sapiens for centuries.. It was the power of survival, the unstoppable urge for change and progress. Then came that day when reports of mysterious disappearances at the borders of the Deeplands reached the population. The two governments of both the Old and New World sent out several reconnaissance groups to investigate these matters at their two borders. Sometimes they disappeared in turn, but after some time it was discovered that in all that time a Third World had emerged. A world that is popularly called the ' Catacombs' because it was underground, in the dark, in the tunnels that swung deep beneath and between the craters. A maze populated by deformed creatures that fed on everything that was edible. Rodents, insects or even worms that had taken foreign and grotesque forms as a result of nuclear violence. The day came when they were 100% sure. The inhabitants of the Catacombs also ate human flesh! In those days, special committees, both from the New World and the Old World, made contact and debated this problem, made solutions and rejected again. What to do without rape nature even more to contain this danger? Because of the fragile peace that prevailed, it was decided to demarcate and guard those specific dangerous zones with man and power. A wall of steel stakes connected to many layers of barbed wire was built around those zones, soldiers patrolled day and night in the vicinity. On both sides there were still reports of disturbing disappearances. One was powerless against the unannounced and bloody nocturnal food raids of the ' Cannibals' , a nickname they were given for obvious reasons. The barbed wire or the sentries didn't always stop them. Sometimes one found the chipped remains of a poor bastard. Sometimes a number of inhabitants of the Catacombs were captured. Prisoners were interrogated and invariably one had to conclude that the underground people were a race with a distinct language that consisted more of hoarse sounds and related gestures. The shelf life of the prisoners was the main problem. After a few days above ground, they invariably had a fever, went into a coma and died in the first week after their arrest.. Some of them committed suicide before it came to that. Much information about the operation of the world of the Cannibals had not yet been collected. In the Old World, the United States of the Western Community pursued a tight security policy. This United States of the Western Community consisted of the former United States of America, South and Central America, Africa and part of Western Europe. One had, of course, the inevitable and deadly problem of radiation. But Japan had experimented in the pre-war years with new means against radiation sickness. Nanoparticles in food and also via injection provided genetic engineering in humans in such a way that at cell level the radiation was less likely to grow into cancer. The research and testing phases were a well-kept industrial secret in the pre-Great War period. An inoculation of both the adults and the younger generation made them much more resilient against this invisible killer. In addition, one had to take his dose of medication daily. The joint operation of both these, the inoculation and the drugs made it possible to live in a world where radiation was a given, probably for a very long time. This technology was unselfishly shared with the Old World today, a very big step in the rapprochement between the two superpowers and the first greater step towards mutual trust. In addition, in the Old World in the post-war period, a technique was developed in which a miniscule identification chip was implanted in everyone. Also an application of nanotechnology. A small routine operation with major consequences. The chip was inserted under the skin at the level of the brain stem. This nanochip paired with the Medulla Oblgata. The connection to the medulla of the brainstem consisted of, on the one hand, microscopic semiconductor material and natural nanotissues to counteract the rejection processes. In the first days after the implantation, these tiny tissues developed fine fibers that sought and connected to very specific parts of the brain such as the amygdala, the hypothalamus and a few other important parts of the brain. These parts gave access to various data stored in the brain during a human life. A groundbreaking discovery in the field of nanotechnology and neurology. This mini-piece of hardware encapsulated after a few days by the attachment of the natural tissues to the brainstem and through the numerous fibers they formed a kind of neuronal connection between various parts of the brain and the nanochip. The introduction of the nanochip into the medulla medulla made it virtually impossible to remove it afterwards without causing irreparable damage to the brain. After the last Revolution in China and Korea, in which scientists and leading representatives of education, both professors and students, this time unanimously supported by the army, had seized power, new values were introduced in these areas. The party administration had died in the aftermath of the nuclear winter except a few or killed during the unrest of the revolution. They were replaced by a committee of leading scientists, students and professors.
